Коронавирусная (лат. Coronaviridae) инфекция COVID-19 (Corona virus disease 2019) – это острая инфекция, вызываемая коронавирусом SARS-CoV-2 (Severe Acute Respiratory Syndrome), которая может протекать в форме острой респираторной инфекции с осложнениями в вирусную пневмонию и острый респираторный дистресс-синдром (Acute Respiratory Distress Syndrome – ARDS).

COVID-19 можно остановить на ранней стадии развития и предотвратить острый респираторный дистресс-синдром (ARDS), который является основной причиной летальных исходов вне зависимости от возраста пациента или сопутствующих хронических заболеваний.

Причина развития острого респираторного дистресс-синдрома (ARDS)

Острый респираторный дистресс-синдром (ARDS) развивается в результате сильного иммунного ответа на вторжение вируса и начало острой диффузной пневмонии.

В начальной (экссудативной) фазе диффузного воспаления легких выделяются противовоспалительные вещества, которые повреждают капилляры и альвеолы, в результате чего эксудат проникает в альвеолярное воздушное пространство, что повреждает легкие и нарушает газообмен.

Активация иммунного ответа вирусом и большой выброс противовоспалительных веществ являются одной из причин острого повреждения легких и развития ARDS.

Профилактика острого респираторного дистресссиндрома (ARDS)

Можно предположить, что уменьшение на ранней стадии активности диффузного воспаления легких позволит уменьшить силу иммунного ответа на вторжение вируса и тем самым позволит предотвратить острый респираторный дистресс-синдром (ARDS).

Так же существует возможность предотвратить активацию иммунного ответа с помощью препаратов – иммуносупрессоров, и сегодня существуют лекарственные препараты – кортикостероиды, которые обладают противовоспалительным действием и способностью угнетать иммунитет в зависимости от дозы и продолжительности их применения.

Многолетнее клиническое наблюдение применения кортикостероидов в соответсвии с действующими рекомендациями демонстрирует несомненный противовоспалительный эффект их использования в малых дозах, а также иммуносупрессорный эффект кортикостероидов в больших дозах в сочетании с их противовоспалительным действием в начальной стадии применения.

Профилактическое применение кортикостероидов в малых дозах сразу после подтверждения заражения или при первых симптомах острой респираторной вирусной инфекции (ОРВИ) за счет их противовоспалительного действия уменьшает вероятность острой диффузной пневмонии, что снижает вероятность сильного иммунного ответа с развитием острого дистресс синдрома.

Например: 1-12 миллиграмм в сутки Триамцинолона в таблетках (1/4-3 таблетки) в течение 1-3-5 дней.

Лечебное применение кортикостероидов в малых дозах, не угнетающих иммунитет, на ранней фазе острой диффузной пневмонии за счет их противовоспалительного действия уменьшает интенсивность и скорость воспалительного процесса, что снижает вероятность сильного иммунного ответа с развитием острого дистресс синдрома.

Например: 4-20 миллиграмм в сутки Триамцинолона в таблетках (1-5 таблеток) в течение 1-3-5 дней.

Иммуносупрессорный и противовоспалительный эффект кортикостероидов

Так же известно, что кортикостероиды в больших дозах при относительно длительном применении) угнетают собственный иммунитет, что позволяет их применять в качестве иммуносупрессоров с мощным противовоспалительным эффектом в начальной фазе из применения.

Например: более 20 миллиграмм в сутки Триамцинолона в таблетках (более 5 таблеток) в течение 1-2 недель и более.

Коррекция дозы кортикостероидов должна производиться ежедневно в сторону увеличения или уменьшения по «клинической потребности».

При угрозе развития вторичной бактериальной инфекции оправдано применение соответствующих антибиотиков.

Нормализация двуокиси углерода(1) в легких имеет противовоспалительный эффект

Значение нормализации константы альвеолярного СО2 (рСО2А(2))

В течение нескольких десятков лет мы наблюдаем несомненный противовоспалительный эффект, наступающий в результате нормализации функции внешнего дыхания и давления СО2 в легких у больных с воспалительными заболеваниями органов дыхания различного характера.

На основании этого можно предположить, что существует возможность на ранней стадии заболевания COVID-19 предотвратить развитие острого диффузного воспаления легких при условии частичной или полной нормализации парциального давления двуокиси углерода (СО2) в альвеолярном воздухе легких (рСО2А) по способу, предложенному к.м.н. К.П.Бутейко более полувека назад.

Многолетнее клиническое наблюдение доказало несомненный противовоспалительный эффект от нормализации этой ключевой константы гомеостаза при лечении смешанных воспалительных заболеваний дыхательных путей.

Например, частичная нормализация рСО2А до 32 миллиметров ртутного столба (при норме в 40 мм рт ст) позволяет за короткое время устранить бронхиальную обструкцию, проявления астмы и отменить лекарственное лечение, что становится возможным в результате снижения активности аллергического воспаления бронхов, обострение которого могло быть вызвано вирусной или микробной инфекцией.

Измерение СО2 в легких

Для измерения альвеолярного СО2 (рСО2А) К.П.Бутейко предложил простой способ, согласно которому продолжительность произвольной задержки дыхания после выдоха, если она выполнена без усилия воли, точно соответствует определенному значению СО2 в легких.

Достоверным показателем отсутствия усилия воли при выполнении задержки дыхания является нормальная глубина или амплитуда первого вдоха после ее завершения.

Продолжительность в секундах задержки дыхания позволяет по специальной таблице Бутейко определить СО2 в легких (в объемных процентах или миллиметрах ртутного столба – мм рт ст).

Продолжительность задержки дыхания больше 45 секунд, если задержка выполнена без усилия воли, соответствует нормальному давлению СО2 в легких (40 мм рт ст), при котором невозможно определить признаки каких-либо воспалительных заболеваний органов дыхания.

Причина дефицита СО2 в легких

Главной причиной уменьшения СО2 в легких (альвеолярной гипокапнии) является гипервентиляция легких(3), устранение которой позволяет достаточно быстро нормализовать константу по СО2 и успешно лечить воспалительные заболевания органов дыхания любого происхождения.

Клиническим признаком избыточной вентиляции легких и альвеолярной гипокапнии являются бронхиальная обструкция(4) возникающая при уменьшении рСО2А менее 28 мм рт. ст. и артериальная гипертензия(5), возникающая в результате периферического ангиоспазма.

Способы нормализации СО2 в легких по Бутейко

С целью устранения гипервентиляции и нормализации СО2 в легких К.П.Бутейко предложил простой способ, в основе которого лежит незначительное уменьшение глубины (амплитуды) дыхания усилием воли с целью создания ощущения легкого недостатка воздуха в течение 1-2 минут.

Так же нормализовать СО2 в легких можно с помощью серии коротких задержек дыхания после выдоха, выполненных с незначительным усилием воли.

Положительным результатом считается устранение негативных симптомов заболевания или несомненное увеличение продолжительности задержки дыхания в течение 5-10 минут упражнения, что говорит о частичном устранении дефицита СО2.

Например, пациенту в качестве упражнения предлагается выполнить серию коротких задержек дыхания после выдоха продолжительностью не более 5 секунд с интервалом не менее 1 минуты.

В нашей практике был случай полного устранения симптомов многолетней бронхиальной обструкции в течение двух недель самостоятельных занятий, когда пациент по рекомендации врача выполнял в течение суток более 200 коротких задержек дыхания после выдоха продолжительностью не более 5-10 секунд каждая.

Другой способ самостоятельных занятий предполагает уменьшение глубины дыхания в легко переносимом объеме с помощью незначительного усилия воли в течение 1-2 минут после короткой задержки дыхания, выполненной после выдоха и не превышающей 5-10 секунд.

Способ лечения, предложенный К.П.Бутейко и основанный на нормализации внешнего дыхания, имеет две особенности:

– устранение гипервентиляции легких осуществляется усилием воли самого пациента, так как, например, вдыхание газовой смеси с высоким содержанием СО2 имеет много нюансов и не дает ожидаемого эффекта по причине сохранение легочной гипервентиляции;

– учитывая определенную техническую сложность нормализации внешнего дыхания в качестве способа лечения, преподаватель методики Бутейко должен иметь подтверждение квалификации от меня лично как соавтора изобретения и правообладателя.

Дыхательная недостаточность(6) при COVID-19 и ARDS: профилактика и лечение

Фрагментарная гибель легких при развитии острого респираторного дистресс-синдрома (ARDS) существенно уменьшает площадь газообмена, тем не менее даже небольшой объём сохранившейся легочной ткани способен обеспечить потребности метаболизма в дыхании при отсутствии элементов бронхиальной обструкции и нормальном рН артериальной крови.

Возникновение бронхиальной обструкции и поддержание рН крови регулируется давлением углекислого газа в легких и крови соответственно.

Гипервентиляция легких, свойственная острой фазе заболевания органов дыхания, создавая дефицит СО2 в легких, провоцирует разную степень бронхиальной обструкции – отёк бронхов, гиперсекрецию слизи и скрытый бронхоспазм, что создает неравномерность вентиляции легких и уменьшает площадь газообмена.

В начальной стадии заболевания дефицит кислорода в клетках (клеточная гипоксия) не связан с повреждением легких или нарушением диффузии газов, поэтому имеет функциональный(7) характер, возникает в результате гипервентиляции легких и сопровождается клеточной, артериальной и альвеолярной гипокапнией – дефицитом углекислого газа в клетках, в крови, в легких.

Артериальная гипокапния изменяет рН артериальной крови и нарушает способность оксигемоглобина к диссоциации(8), что провоцирует клеточную гипоксию.

На этом этапе заболевания устранение гипервентиляции легких по способу, предложенному Бутейко, позволяет нормализовать газообмен и устранить клеточную гипоксию без особенных трудностей.

В противном случае хроническая гипервентиляция легких и альвеолярная гипокапния будут причиной бронхиальной обструкции и снижения эффективности газообмена, а последующее повреждения легких в результате сильного иммунного ответа на вторжение вируса окажется причиной клеточной и артериальной гиперкапнии(9) и гипоксии.

На этом этапе заболевания критически важно нормализовать внешнее дыхание и устранить гипервентиляцию лёгких по способу, предложенному Бутейко, что позволит устранить бронхиальную обструкцию и нормализовать газообмен через неповрежденные участки легких.

Устранение бронхиальной обструкции нормализует газообмен через здоровые участки легких и позволяет эффективно бороться с гиперкапнией, а последующая нормализация рН крови нормализует диссоциацию оксигемоглобина, что позволяет успешно бороться с клеточной гипоксией у больных с обширным поражением легких, у больных с обширным пневмосклерозом(10) или тяжелой формой бронхиальной астмы.

Ограничение эффективности искусственной вентиляции легких (ИВЛ)

ИВЛ применяется для компенсации дыхательной недостаточности, нормализации газовых констант и дыхательного гомеостаза(11).

На фоне острой гибели легких в результате иммунного ответа на вторжение вируса и развития клеточной и артериальной гиперкапнии, применение ИВЛ не должно сопровождаться хронической гипервентиляцией легких.

В противном случае на фоне острой гиперкапнии стойкий дефицит альвеолярного СО2 окажется причиной обширной бронхиальной обструкции, неравномерности вентиляции, дополнительного изменения рН крови и распада оксигемоглобина с последующим нарастанием острой тканевой гипоксии, что может повлечь за собой в качестве компенсаторной реакции дыхательного гомеостаза ускоренное развитие пневмосклероза.

Ограничение эффективности кислородной терапии

Дыхание газовой смесью с повышенным содержанием кислорода имеет своей целью увеличить насыщение артериальной крови и клеток организма кислородом.

Однако, известно, что кровь всегда и при любых условиях полностью насыщена кислородом, даже при значительном уменьшении площади вентилируемой поверхности легких, и весь гемоглобин, который является физическим носителем кислорода, занят.

Отсутствие свободного гемоглобина не позволяет увеличить процент насыщения крови кислородом, а незначительный клинический эффект может быть обусловлен повышением кислорода в плазме крови.

Решение проблемы дыхательной недостаточности

Оказывается, что для устранения клеточной гипоксии, артериальной гипоксемии(12), клеточной и артериальной гиперкапнии достаточно нормализовать внешнее дыхание по способу, предложенному К.П.Бутейко.

При отсутствии повреждения легких газообмен нормализуется в результате устранения элементов скрытой бронхиальной обструкции, нормализации рН крови и нормальной диссоциации оксигемоглобина.

При повреждении легких газообмен нормализуется через здоровые участки легочной ткани и окажется достаточным для нормального метаболизма при условии отсутствия скрытой бронхиальной обструкции и нормального рН артериальной крови.

SARS-CoV-2 и COVID-19: профилактика заражения

Не смотря на высокую контагиозность(13) вируса, тем не менее, дыхание через нос является природным, трудно преодолимым барьером для вирусной и микробной инфекции

Главное правило профилактики инфекционных заболеваний – это остановить передачу инфекции, разорвать путь передачи возбудителя.

Следует помнить о необходимости дышать исключительно через нос и в покое, и в во время физической активности, особенно во время занятий спортом.

Нос выполняет три важнейших функции: воздух согревается, воздух обеззараживается, воздух увлажняется.

Многолетний анализ случаев заболеваемости убедительно показал, что главной причиной распространения вирусной инфекции является дыхание через открытый рот.

Так, например, у детей в процессе лечения бронхиальной астмы по способу, предложенному Бутейко, формируется стойкая привычка дышать через нос, особенно при выполнении физической нагрузки, что снижает за короткое время на 50% число случаев заболеваний острой респираторной вирусной инфекцией (ОРВИ) и гриппом.

(7) Функциональное нарушение – возникает при изменении различных параметров без органических повреждений, имеет обратимый характер.

(8) Диссоциация оксигемоглобина – способность оксигемоглобина распадаться, отдавая кислород в клетки в зависимости от рН крови.

(9) Гиперкапния – избыточное содержание СО2.

(10) Пневмосклероз, пневмофиброз – рубцовые изменения в легких в результате их гибели.

(11) Дыхательный гомеостаз – постоянные величины дыхательных газов (О2, СО2) в клетках, крови, легких.

(12) Гипоксемия – уменьшение кислорода в крови.

(13) Контагиозность – свойство инфекции передаваться от больных к здоровым.

 

COVID-19: prevention and treatment

Coronavirus (lat. Coronaviridae) infection COVID-19 (Corona virus disease 2019) is an acute infection caused by the SARS-CoV-2 coronavirus (Severe Acute Respiratory Syndrome), which can occur in the form of an acute respiratory infection with complications in viral pneumonia and acute respiratory distress syndrome (Acute Respiratory Distress Syndrome – ARDS).

COVID-19 can be stopped at an early stage of development and prevent acute respiratory distress syndrome (ARDS), which is the leading cause of death, regardless of the patient’s age or associated chronic diseases.

The cause of Acute Respiratory Distress Syndrome (ARDS)

Acute respiratory distress syndrome (ARDS) is developed as a result of a strong immune response to virus invasion and the onset of acute diffuse pneumonia.

In the initial (exudative) phase of diffuse pneumonia, anti-inflammatory substances are released that damage the capillaries and alveoli. As a result the exudate penetrates the alveolar air space, which damages the lungs and disrupts gas exchange.

Activation of the immune response by the virus and high release of anti-inflammatory substances are one of the causes of acute lung damage and the development of ARDS.

Prevention of Acute Respiratory Distress Syndrome (ARDS)

It can be assumed that a decrease in the early stage of diffuse pneumonia activity will reduce the strength of the immune response to virus invasion and thereby prevent acute respiratory distress syndrome (ARDS).

There is also an opportunity to prevent the activation of the immune response with the help of drugs – immunosuppressants, and today there are drugs – corticosteroids, which have antiinflammatory effects and the ability to inhibit the immune system depending on the dose and duration of their use.

Long-term clinical observation of the use of corticosteroids in accordance with the current recommendations demonstrates the undoubted anti-inflammatory effect of their use in small doses and the immunosuppressive effect of corticosteroids in large doses in combination with their anti-inflammatory effect in the initial stage of use.

Due to the anti-inflammatory effect the prophylactic use of corticosteroids in small doses immediately after confirmation of infection or at the first symptoms of acute respiratory viral infection (ARVI) reduces the likelihood of acute diffuse pneumonia and therefore reduces the likelihood of a strong immune response with the development of acute distress syndrome.

For example: 1-12 milligrams per day of Triamcinolone in tablets (1/4-3 tablets) for 1-3-5 days.

The therapeutic use of corticosteroids in small doses that do not inhibit immunity in the early phase of acute diffuse pneumonia (due to their anti-inflammatory effect) reduces the intensity and speed of the inflammatory process, which reduces the likelihood of a strong immune response with the development of acute distress syndrome.

For example: 4-20 milligrams per day of Triamcinolone in tablets (1-5 tablets) for 1-3-5 days.

Immunosuppressive and anti-inflammatory effect of corticosteroids

It is also known that high doses of corticosteroids with relatively long-term use inhibit our immunity. It allows to use them as immunosuppressants with a powerful anti-inflammatory effect in the initial phase of use.

For example: more than 20 milligrams per day of Triamcinolone in tablets (more than 5 tablets) during 1-2 weeks or more.

Correction of the dose of corticosteroids should be made daily in the direction of increase or decrease according to “clinical need”.

If there is a risk of secondary bacterial infection, the use of appropriate antibiotics is justified.

Normalization of carbon dioxide in the lungs has an anti – inflammatory effect

The value of normalization of the constant alveolar CO2 (pCO2A)

Over the course of several decades, we have been observing an undoubted anti-inflammatory effect resulting from the normalization of external respiration function and CO2 pressure in the lungs in patients with various types of inflammatory respiratory diseases.

Based on this, it can be assumed that, at an early stage of COVID-19 disease, it is possible to prevent the development of acute diffuse pneumonia under the condition of complete normalization of the partial pressure of carbon dioxide (CO2) in the alveolar air of the lungs (pCO2A) according to the method proposed by the Dr. Konstantin P. Buteyko (MD, PhD) more than half a century ago.

Long-term clinical observation proved the undoubted anti-inflammatory effect of the normalization of this key constant of homeostasis in the treatment of mixed inflammatory diseases of the respiratory tract.

For example, partial normalisation of pCO2A to 32 millimetres of mercury (at a norm of 40 mm Hg) allows in a short time to eliminate bronchial obstruction, asthma and to cancel drug treatment. It becomes possible as a result of a decrease in the activity of allergic inflammation of the bronchi, which could be aggravated by viral or microbial infection.

CO2 measurement in the lungs

To measure alveolar CO2 (pCO2A), Dr. K. Buteyko proposed a simple method, according to which the duration of a spontaneous delay in breathing after exhalation (if performed without volitional effort) exactly corresponds to a certain value of CO2 in the lungs.

Carbon dioxide, carbon dioxide, CO2 – one of the respiratory gases, whose constant value maintenance in the cells, in the blood, in the lungs along with oxygen (O2) ensures the performance of the functional respiratory system. The normal content of CO2 in the lungs is ensured by metabolic activity and the functioning of the external respiration system.

The movement of respiratory gases is the main regulator of the acid-base balance (pH) of the body.  pCO2A is the partial pressure of carbon dioxide in the alveolar air of the lungs.

A reliable indicator of the lack of the volitional effort when holding a breath is the normal depth or the normal amplitude of the first inhale after it is completed.

The duration in seconds of breath holding allows to determine CO2 in the lungs according to a special Buteyko table (in volume percent or millimetres of mercury – mm Hg).

The duration of breath holding of more than 45 seconds, if it is performed without volitional effort, corresponds to the normal pressure of CO2 in the lungs (40 mmHg), and then it is impossible to determine any signs of inflammatory disease of the respiratory system.

The cause of CO2 deficiency in the lungs

The main reason for the decrease in CO2 in the lungs (alveolar hypocapnia) is pulmonary hyperventilation , the elimination of which allows you to quickly normalise the constant of CO2 and successfully treat inflammatory diseases of the respiratory system of any origin.

A clinical sign of excessive ventilation of the lungs and alveolar hypocapnia are bronchial obstruction4 resulting from a decrease in pCO2A of less than 28 mm Hg and arterial hypertension resulting from peripheral angiospasm.

The methods of CO2 normalisation in the lungs using Buteyko

In order to eliminate hyperventilation and normalise CO2 in the lungs, Konstantin. P. Buteyko proposed a simple method, based on a slight decrease in the depth (amplitude) of breathing with volitional effort in order to create a feeling of a slight lack of air for 1-2 minutes.

It is also possible to normalise CO2 in the lungs using a series of short breath holdings after exhalation, performed with a slight volitional effort.

A positive result is the elimination of negative symptoms of the disease or an undoubted increase in the duration of breath holding during 5-10 minutes of exercise, which indicates a partial elimination of CO2 deficiency.

For example, the patient is offered to perform a series of short breath holdings of not more than 5 seconds after exhalation with an interval of at least 1 minute.

In our practice, there was a case of the complete elimination of the symptoms of the long-term bronchial obstruction during two weeks of self-tuition, when the patient, on the recommendation of a doctor, performed over 200 short breath-holdings of not more than 5-10 seconds each after exhalation during the day.

Hyperventilation of the lungs – an increase in general ventilation of the lungs in relation to the needs of metabolism. It is 3 determined by the development of deficiency of the final metabolic product – CO2, which is removed through the lungs during breathing.

Bronchial obstruction – a decrease in the lumen of the bronchi as a result of edema, excessive sputum and spasm (compression). In some diseases, it is reversible.

Hypertension – an increase in blood pressure, most often occurs as a result of compression of peripheral vessels 5 (peripheral angiospasm).

Another way of Buteyko self-studying implies reducing the depth of breathing in an easily tolerated volume with the help of the slight effort within 1-2 minutes after a short breath hold not exceeding 5-10 seconds and performed after exhalation.

The treatment method proposed by Dr. K. Buteyko and based on the normalisation of external respiration has two features:

A / elimination of hyperventilation of the lungs is carried out by the patient himself using volitional effort, since, for example, inhaling a gas mixture with a high content of CO2 has many nuances and does not give the expected effect due to the preservation of pulmonary hyperventilation;

B / given the certain technical complexity of normalising breathing as a method of treatment, the teacher of the Buteyko method should have the confirmation of his/her proficiency and qualification directly from me as a co-author of the invention and copyright holder.

Respiratory failure in COVID-19 and ARDS: prevention 6 and treatment

Fragmentary death of the lungs during the development of acute respiratory distress syndrome (ARDS) significantly reduces the gas exchange area. Nevertheless, even a small amount of preserved lung tissue can satisfy the metabolic needs for breathing in the absence of elements of bronchial obstruction and normal pH of arterial blood.

The occurrence of bronchial obstruction and the maintenance of blood pH is regulated by the pressure of carbon dioxide in the lungs and blood, respectively. Hyperventilation of the lungs, a characteristic feature of the acute phase of respiratory disease, creating a deficiency of CO2 in the lungs, provokes a different degree of bronchial obstruction – edema of the bronchi, hyper secretion of mucus and latent bronchospasm, which creates uneven ventilation and reduces the area of gas exchange.

In the initial stage of the disease, oxygen deficiency in cells (cellular hypoxia) is not associated with lung damage or disturbance of gas diffusion, therefore it has functional aspect. Elimination of bronchial obstruction normalises gas exchange through the healthy areas of the lungs and allows you to effectively fight hypercapnia; and subsequent normalisation of blood pH normalises the dissociation of oxyhemoglobin, and it allows you to successfully fight cell hypoxia in patients with extensive lung damage, in patients with extensive pneumosclerosis or severe 10 bronchial asthma.

Limiting the effectiveness of mechanical ventilation

Mechanical ventilation is used to compensate for respiratory failure, normalise gas constants and respiratory homeostasis . 11

With the acute lung death as a result of an immune response to virus invasion and the development of cellular and arterial hypercapnia, the use of mechanical ventilation should not be accompanied by chronic hyperventilation of the lungs.

Otherwise, with the acute hypercapnia, a persistent deficiency of alveolar CO2 will cause extensive bronchial obstruction, uneven ventilation, additional changes in blood pH and the breakdown of oxyhemoglobin, followed by an increase in acute tissue hypoxia. It may lead to the accelerated development of pneumosclerosis as a compensatory reaction of respiratory homeostasis.

Limiting the effectiveness of oxygen therapy

The breath of a gas mixture with a high oxygen content is intended to increase the saturation of arterial blood and body cells with oxygen. However, it is known that blood is always and under any conditions completely oxygenated, even with a significant diminished area of the ventilated surface of the lungs, and all hemoglobins, which is a physical carrier of oxygen, is occupied.

Resolving the problem of respiratory failure
